What is a Sliding Fee Discount Program?

The Sliding Fee Discount Program (SFDP) helps make healthcare services more affordable for uninsured, underinsured, and low-income individuals. This program adjusts the cost of care based on household income and family size, ensuring that those with lower incomes pay reduced fees for the same high-quality services available at standard rates.

If approved for the SFDP, your discount will be valid for one year. To continue receiving reduced-cost services, you must reapply annually.

To qualify for the Sliding Fee Discount Program (SFDP), you must provide proof of gross income for all household members. Gross income includes all earnings before taxes.

Acceptable proof of income includes any of the following:

  • Previous year’s federal tax return (showing gross income)
  • One month’s paycheck stubs
  • Unemployment compensation verification
  • Disability benefits verification
  • Pension or retirement income
  • Educational assistance
  • Alimony or child support
  • Worker’s compensation
  • Supplemental income
  • Veteran’s or survivor’s benefits
  • Interest, dividends, rental income, royalties, or estate funds
  • A signed letter from an employer on company letterhead (confirming gross wages)
  • A notarized letter of support from a third party (confirming how they are supporting you)
  • Any other source of income
