What is a Sliding Fee Discount Program?

The Sliding Fee Discount Program (SFDP) is designed to provide healthcare services to uninsured, underinsured, or low-income individuals. The program’s goal is to adjust the cost of healthcare services for qualified patients based on their household income and size. As a result, patients in lower-income households will pay less for the same healthcare services that are offered at an undiscounted cost. If you are approved for the SFDP, your discount will be valid for one year. To continue receiving the discount, you must reapply each year.

To qualify for the Sliding Fee Discount Program, you must show proof of gross income for ALL household members. Gross income is all income before taxes. Acceptable proof of income is any of the following: 

  • Previous year’s federal tax return showing Gross Income 
  • One month’s paycheck stubs 
  • Unemployment compensation verification 
  • Disability benefits verification 
  • Pension or retirement income 
  • Educational assistance 
  • Alimony 
  • Child support 
  • Worker’s compensation 
  • Supplemental income 
  • Veteran’s or Survivor’s benefits 
  • Interest 
  • Dividends 
  • Rental income 
  • Royalties and estate funds 
  • A signed letter from employer on company letterhead confirming gross wages 
  • A notarized letter of support from a third party confirming how they are supporting you.
