Full-time position 6-month Pilot Program (07/01-12/31). Will decide if/how to continue by 12/02/2025. (Shift 12:30 pm – 8:30 pm M-F or 10:30 am – 8:30 pm M-Th TBD by hire date)
Position Summary: Amador Health Center is seeking a compassionate and skilled Family Nurse Practitioner to deliver quality primary health care at our Federally Qualified Health Center (FQHC). Our target population includes adults, children, and families experiencing homelessness. We prefer candidates with a minimum of three years of experience as a Family Nurse Practitioner. The ideal candidate will be a collaborative partner committed to our mission and dedicated to improving our quality of care and patient health outcomes. Bilingual and entire lifespan experience strongly desired.
Major Activities and End Results:
- Provide patient care to adults and children at the clinic and during outreach activities.
- Participate actively in multi-disciplinary teams, including outreach, Quality Improvement (QI), and patient health education.
- Support the Medical Director in ensuring efficient clinic operations.
- Mentor junior providers or interns as needed.
- Assist the clinical team with compliance regarding HIPAA, OSHA, and other health and safety regulations.
- Collaborate with the Quality Improvement team on developing projects and report to the CMO on issues related to quality care, including behavioral health and dental care.
Minimum Qualifications: Experience working with underserved populations and providing primary care in a clinical setting.
Desired Qualifications: Bilingual English/Spanish preferred. Experience with chronic health issues and co-morbidities in underserved populations. Multi-year hands-on experience as a Nurse Practitioner. Collaborate with the Quality Improvement team on developing projects and report to the CMO on issues related to quality care, including behavioral health and dental care.
Education, Credentials, Licenses: Certified Nurse Practitioner, NM Board of Pharmacy Controlled Substances, and DEA certification
Other Requirements: State-issued driver’s license, Criminal background check, Pre-employment drug test Collaborate with the Quality Improvement team on developing projects and report to the CMO on issues related to quality care, including behavioral health and dental care.
Amador Health Center is a National Health Service Corps site, and employment may be eligible for loan repayment.
