We are seeking a Case Manager to join our Behavioral/Mental Health Program. You’ll assess client needs, develop care plans, and advocate for clients to ensure they receive the resources they need to thrive.

Key Responsibilities:

  • Assess clients’ mental health needs and develop and implement personalized care plans.
  • Coordinate services between providers and agencies.
  • Offer specialized support to LGBTQ patients.
  • Monitor client progress, adjust care plans, and assist with therapy sessions.
  • Advocate for clients to access resources
  • Maintain accurate client records.
  • Provide crisis intervention and support.
  • Educate clients and families about mental health.
  • Collaborate with multidisciplinary teams.

Minimum Qualifications:

  • Bilingual Spanish and English.
  • Knowledge of community services and resources.
  • Understanding of mental health conditions and LGBTQ issues.
  • Excellent communication and organizational skills.
  • Compassionate, empathetic, and strong advocacy skills.

Preferred Qualifications:

  • Degree in social work, psychology, counseling, or related field.
  • Experience with electronic health records (EHR), preferably Athena.
