We are seeking a Case Manager to join our Behavioral/Mental Health Program. You’ll assess client needs, develop care plans, and advocate for clients to ensure they receive the resources they need to thrive.
Key Responsibilities:
- Assess clients’ mental health needs and develop and implement personalized care plans.
- Coordinate services between providers and agencies.
- Offer specialized support to LGBTQ patients.
- Monitor client progress, adjust care plans, and assist with therapy sessions.
- Advocate for clients to access resources
- Maintain accurate client records.
- Provide crisis intervention and support.
- Educate clients and families about mental health.
- Collaborate with multidisciplinary teams.
- Bilingual Spanish and English.
- Knowledge of community services and resources.
- Understanding of mental health conditions and LGBTQ issues.
- Excellent communication and organizational skills.
- Compassionate, empathetic, and strong advocacy skills.
Preferred Qualifications:
- Degree in social work, psychology, counseling, or related field.
- Experience with electronic health records (EHR), preferably Athena.
